          Clinical trials to assess adjuvant therapeutics for severe malaria

          The current model for assessing adjunctive therapies for severe malaria (i.e. randomized controlled trials) is limited by the large number of participants required to reach mortality endpoints. This barrier requires innovative solutions including the use of surrogate markers of mortality. In this opinion piece, we examine the use of immune and endothelial activation markers involved in the pathobiology of severe malaria, as potential surrogate markers of mortality. In addition, we highlight the importance of risk-stratifying participants with these same biomarkers and the potential of repurposing drugs active on those pathways to accelerate the identification of effective adjunctive therapeutics.
